Million Hearts® is a national initiative to prevent one million heart attacks and strokes by 2017. To reach this ambitious goal, Million Hearts has brought and continues to bring together existing efforts and new programs to improve community health.
The CDC Million Hearts® Collaboration coordinates and disseminates priority messages and strategies related to the Million Hearts initiative. The American Heart Association and the National Forum for Heart Disease & Stroke Prevention are co-leading the Collaboration, which includes the Association of State and Territorial Health Officials, the National Association of Chronic Disease Directors with its Cardiovascular Health Council, and the National Association of County and City Health Officials.

With our CDC Million Hearts Collaboration partners and National Forum members, the National Forum created tools and resources organizations can use to advance the Million Hearts goal to prevent one million heart attacks and strokes by 2017.
