Each year, the National Forum recognizes people and groups that have helped advance heart disease and stroke prevention above and beyond the norm. The dedication, commitment, and perseverance shown by this year’s awardees is truly inspiring. The work that these people and organizations have done has saved lives; and they have all advanced the recommendations within the Public Health Action Plan to Prevent Heart Disease and Stroke. Presented to an individual and group in the field of public health whose work embodies the recommendations of the Public Health Action Plan to Prevent Heart Disease and Stroke.
