Novel Data-Driven Approaches

The 2024 annual meeting of the National Forum for Heart Disease and Stroke Prevention will raise awareness and fluency in the newly developed Vital Conditions for Health and Wellbeing (Vital Conditions) framework among medical and public health practitioners and researchers. It will also ensure that discussions on health equity and its implications for cardiovascular disease are framed within the Vital Conditions principles and recommendations.

  • Presentations on the importance of using data-driven approaches:
    • Ways of improving health policy and healthcare systems.
    • How to increase the perception of health equity and actions at community and individual levels.
    • Examine the relatively new Vital Conditions framework, which expands on the social determinants of health paradigm that has long dominated health equity discourse.
      • The Vital Conditions model is intended to make policies, programs, and environmental changes that improve health equity more appealing to more people.
      • Address the structural drivers of inequities, the framework tracks how individual needs form an interconnected web that helps people and places thrive collectively.
      • Recognizing that when a vital condition goes unfulfilled, it can compound other existing conditions, including individual health and well-being outcomes. An important vital condition addition, civic belonging, is an opportunity to engage communities and provide long-term impact.
      •  Vital Conditions will help meet emerging performance requirements and goals, as well as new regulatory requirements and accreditation standards.
